Insurance and Payment Information
We are an in-network provider for Blue Cross Blue Shield of Illinois PPO plans and will bill directly for all eligible services. Any additional payments such as coinsurance, copayments or payments toward a deductible are expected at the time of service.
We are an out-of-network provider for all other insurance plans. A statement will be provided to you if you wish to pursue reimbursement with your insurance company. Please contact your insurer directly to determine out-of-network coverage and benefits. Some helpful questions that may be helpful to ask include:
- Do I have out-of-network mental health benefits?
- What is my out-of-network deductible and what portion of this has been met?
- Is there a limit to the number of sessions/office visits per year?
- Do I have a copayment and if so what is it?
- Do I have a coinsurance payment and if so what is it?
Payment is required at the time of service and may be made via cash, check, credit card, Health Savings Account (HSA) debit and electronic payments including Venmo and Apple Pay.
